It is obvious to me that at the time, it was only Kate who was concerned about the twins. The remark made by Fiona in her statement when she said 'that was another thing, she kept going back and placing her hand on the backs of the twins' she found it odd for Kate to do that. So Fiona thought even though the twins didn't wake with all the noise that they were fine. No doubt she was telling Kate they were fine. It was only Kate who found the stain on Madeleine's pyjama's and only Kate who was wondering if the abductor had tried the night before.
Kate was also in a state of shock obviously, her daughter was missing, having not been in that position I can't even begin to imagine what state her head was in.
With being consoled and checking the twins, Kate still asked the PJ if they could be tested for drugs, just in case probably. Kate must have known the twins were ok and just sleeping.
Later though when speaking to Gerry in my opinion they both wanted the twins tested.
In my opinion this request was reported to Amaral who did nothing. Then when writing his book made up some ludicrous reason why he didn't have the twins tested.
This is the whole quote from Fiona's rogatory inteview :

'No, and that was the other thing, she kept going into the twins, she kept putting her hands on the twins to check they were breathing, she was very much concerned in checking that they were okay. But they were okay, I mean, they were fine, they didn't, they were asleep, but at the time it did seem weird, I remember thinking, you know, when the Police came they turned the lights on, there was loads of noise, obviously from the moment Kate discovered that Madeleine was gone, the screaming and the shouting and there was a lot of noise and they, they didn't, you know, so much as blink'.
So it 'seemed weird ' but as a doctor she did nothing either. That's what's really weird.
